Paula Bland, AGNP-BC, is a board-certified adult-gerontology primary care nurse practitioner at FMOL Health | St. Dominic. She earned her master of science in nursing from the University of Mississippi School of Nursing and has experience caring for adults in primary care, transitional care and hospital settings. 

Paula provides preventive care and treatment for acute and chronic health conditions, with a focus on patient education and long-term disease management. Her experience also includes developing and supporting an at-home transitional care program designed to help patients manage their health after hospitalization and reduce the risk of readmission.  

She works closely with patients, families and other healthcare professionals to coordinate care that supports each patient’s needs and health goals.
